Rheumatology appointment checklist

A rheumatology review is short and the gaps between them are long. This page is what you bring into the room: your questions at the top, the period since the last review condensed underneath.

It follows the structure in the rheumatology appointment guide, which explains each section.

How to use it

  1. Print it, or fill it in on screen a day or two before the appointment.
  2. Write your two or three most important questions first. Everything else supports them.
  3. Use your diary or app to fill in the summary for the whole period, not the last fortnight.
  4. In the room, lead with the questions and offer the rest as detail.
