Doctor appointment symptom summary

This is the general version of the appointment checklist — for a GP, a physiotherapist, a pain clinic or any specialist. It condenses a period of tracking into the four things a clinician can act on in a short slot.

The pain diary guide and the appointment guide explain how to fill each section from a record.

How to use it

  1. Fill it in from your diary or app, looking at the whole period rather than the last two weeks.
  2. Keep observations and conclusions apart: "worse mornings tended to follow short nights" is an observation.
  3. Lead with your questions in the appointment; offer the rest as detail.
