Flare tracking template

Flares are vivid at the time and blurry afterwards. A card filled in at the start and again at the end turns "quite a few, lasting a while" into a count and a duration a clinician can use.

How to use it

  1. Start a card on the day you would call a flare beginning. Estimate the date if you are late, and say so.
  2. Fill in the "before" section from your diary, not from memory, and without deciding what caused it.
  3. Keep your daily diary going through the flare; the card is a summary, not a replacement.
  4. Close the card on the day it settles. Then leave it alone until you have several to compare.
