Ankylosing spondylitis symptom diary template

A diary that asks too much gets abandoned. This one asks for five numbers and a tick each morning, with a single line for anything unusual.

The columns mirror the structure of the BASDAI, so a monthly score is a glance at the week rather than a guess. The symptom diary guide explains the choices.

How to use it

  1. Print one page per week.
  2. Fill it in each morning once your stiffness has eased — that is when duration is known.
  3. Score pain and stiffness 0–10 for the day so far, and stiffness duration in minutes.
  4. Tick the flare column on days you would call part of a flare; note the start date in the margin.
  5. Once a month, read the pages back for direction and for the worst stretch before you look for anything else.
